💙 Who Can Access This Program?

Our Hospital Discharge Support Program is designed for:
• NDIS participants preparing for hospital discharge who need continued care and assistance.
• Individuals requiring Medium Term Accommodation (MTA), Short Term Accommodation (STA), or Specialist Disability Accommodation (SDA) while awaiting permanent housing.
• Those needing ongoing daily living support or rehabilitation assistance during their recovery period.

🏠 How the Program Works

Our approach focuses on person-centred care, ensuring every participant’s recovery plan is unique to their goals and needs.
1. Collaborative Care Planning
We coordinate with hospital discharge teams, families, and allied health professionals to design a personalised support plan. This includes selecting the right accommodation and planning your daily and therapeutic support needs.
2. Medium-Term Accommodation (MTA)
ADC provides safe, comfortable, and NDIS-funded temporary housing options while participants prepare to transition into their long-term home.
3. 24/7 Personalised Support
Our trained support workers are available around the clock to assist with personal care, mobility, meal preparation, medication management, and emotional wellbeing.
4. Pathway to Permanent Living (SDA)
While participants stay in temporary housing, we help identify suitable Specialist Disability Accommodation that offers long-term comfort, accessibility, and independence.
5. Continuous Monitoring & Adjustment
We regularly review each participant’s progress, adjusting their care plan as their needs evolve. Our allied health team and support coordinators remain engaged throughout the journey.
